This is Post Reflexology Treatment Notes for Clients. This form is to use after giving a client Reflexology Treatment and is something the client takes home with them. The client can fill it in and bring back when they re-visit. It gives you valuable information therefore, for future visits. IMPORTANT PLEASE NOTE THAT YOUR FORM(S) WILL BE SENT TO YOU VIA EMAIL AT THE EMAIL ADDRESS YOU USE TO PAY PAYPAL UNLESS YOU LET ME KNOW IN A SEPERATE EMAIL OF THE EMAIL ADDRESS YOU WOULD PREFER IT/THEM SENT TO.
