About sleep patterns and stages of sleep.
Sleep is defined as a function of the brain. The body rests but the brain sleeps. A healthy young adult good sleeper has 95% sleep efficiency. That is, 5% or less of the total time in bed is spent awake. Sleep onset is swift (less than 15 minutes) and awakenings are few and brief.
There are two phases of sleep depending upon whether or not the eyes can be seen to move behind the closed eyelids. These stages are NREM (nonrapid eye movement) sleep and REM eye movement) sleep. NREM is itself divided into four stages. These sleep stages have been defined according to polysomnographic recordings, chiefly electroencephalograms (EEG). Stage 1 sleep is the transition from drowsy wake to sleep, and stage 2 is light sleep. Stages 3 and 4 are deep sleep, also known as slow wave or delta sleep. REM (rapid eye movement) sleep occurs in sleep stage 5 in which dreaming occurs. Sleep stages occur in cycles throughout the night.
Sleep patterns change with age. Generally there is a gradual decline in overall total sleep time with the elderly spending more time in bed but less time sleeping. After middle age there can be fragmented sleep; that is greater wakefulness intermixed with sleep. Some portion or age-associated deterioration in sleep may simply be related to the aging process. However sleep can also be compromised by related pathophysiology, such as arousals from sleep apnea.
There are three basic mechanisms coordinating and governing sleep and wakefulness. They are: (1) autonomic nervous system balance, (2) homeostatic sleep drive and (3) circadian (daily) rhythms. Sleep requires a transition in the autonomic nervous system to decreased sympathetic activity. Thus both drinking a cup of coffee and anxiousness can keep one awake at night through autonomic mechanisms. Homeostatic regulation of sleepiness is similar to that for thirst, hunger and sex. Continuous uninterrupted, prolonged sleep deprivation eventually makes sleep irresistible. Sleep loss diminishes coping skills; increases irritability and ease of frustration. Sleep deprivation also increases levels of the stress hormone cortisol. The sleep-wake circadian rhythm governs the feelings of alertness and sleepiness during different times of day.
Insomnia is defined as difficulty falling asleep, difficulty sleeping through the night, frequent night awakenings, early awakening or poor quality /unrefreshing sleep and usually leads to impaired functioning the next day. Insomnia is most commonly a transient problem of less than two weeks, and has been estimated to affect up to 80% of the population in the US. Chronic insomnia has been estimated to affect 15% of the population.
Primary insomnia is a sleeping problem not associated with any other health problem. Secondary insomnia is related to a health condition such as depression, heartburn, cancer, asthma, pain, or related to administration of a medication or alcohol.
