Quarto, sometime rebound modern brown morocco,

pp. [xvi], 890, [10].  Title-page & Dedication leaf torn & repaired with some loss of text; some damp-staining to lower corners of next 20 leaves & last 5 pages of the Index missing but supplied in manuscript by a 17th century hand. Size: 19 x 13 x 6cm.
