BY LAW ALL BUSINESSES MUST HAVE AN ACCIDENT REPORT BOOK

First of all lets hope you never need to fill a form in!
But if you do this book has 48 forms which comply with the Data Protection Act 1998 as this book has been designed to facilitate easy removal for storage of personal information (Shaded green area as shown in the open page picture)

Inside front cover gives information and advice to both employee & employer as to how to fill in the forms.

Book Closed size is H=244mm x W=174mm
(Smaller than A4 but larger than A5)
